1.     That I am the executant of this Affidavit/Undertaking and fully conversant with the facts as stated hereunder.
2.     That I have started a business enterprise within the jurisdiction of _____ with the name of _____ Company, having its principal office at _____.
3.     That as required under the laws of _____, I was required to have a national as my sponsor and contacted _____, a national of _____ having I.D. No. _____ and presently working at _____. Accordingly, _____ introduced me to _____, national of _____ with I.D. No. _____, to assist me in setting up my Company.
4.     That _____ was kind enough to assist me in setting up the company and became my sponsor. However, _____ was not in any manner involved in the working and/or affairs of the Company, nor responsible for business obligations/commitments either on my behalf or on behalf of the Company, nor responsible for any loss or profits to the Company, its shareholders, stakeholders, or third persons.

5.     That _____ merely introduced me to my local sponsor and is not at all responsible in any manner for any financial commitments to the Company, its shareholders, stakeholders, and/or any third persons either in _____ or any other part of the world.
6.     That I fully undertake, confirm, and declare that _____ is not in any manner involved in the Company except the facts stated hereinabove.

I solemnly affirm hereunder that in case any claim, dispute, or financial obligations occur or arise as a result of 

loss to the Company, its shareholders, stakeholders, or any other third person, _____ shall not be held responsible in any manner whatsoever, and it will be the sole responsibility of the Deponent.
7.     That I further undertake and indemnify _____ against all losses, financial obligations, claims, disputes in case any arise as a result of any mis‑commitment and breach of financial obligations from the Deponent to the Company, its shareholders, stakeholders, and/or third persons.
8.     That I further affirm on oath the acceptability and enforceability of this Affidavit/Undertaking before _____ Court, Office of the Public Prosecutor, and/or any other office of _____.
9.     That whatever stated hereinabove is true and correct to the best of my knowledge and belief.
